LORATADINE
Mechanism of action
It is a piperidine
derivative and is a long acting selective peripheral H1 antagonist which lacks
CNS depressant effects and is faster acting than astemizole.
It is rapidly, absorbed, not metabolised and is
excreted unchanged primarily in urine. In contrast to terfenadine
and astemizole, it has not produced cardiac
arrhythmia in overdose. Action starts in 1-3 hours. It lasts for 24 hours.
Indications
Allergic skin disorders specially atopic dermatitis and
urticaria. Allergic rhinitis
Dosage
Adults
10mg once a day.
Children
Less than 2yrs
: Not recommended; 2-12 years: 5mg if less than 30kg
; 10mg if more than 30kg
Contraindications
Hypersensitivity
Special precaution
Caution in patients with renal
impairment and elderly.
Adverse drug reactions
Nausea,
epigastric distress. Fatigue and light
headedness
Status in
1. Pregnancy
: Use with extreme caution.
2.
Lactation : Contraindicated
3. Old
age : Use in reduced dose.
4.
Children : Use with extreme caution.
Interactions
Cimetidine, ranitidine
, theophylline, macrolide
antibiotics and ketoconazole, : Though they inhibit hepatic metabolism of loratadine, no clinically significant interaction is noteded.
